Drop nameserver handling from ip= boot parameter.

Debian's live-boot added support for handling nameserver
option inside the ip= boot parameter.

This violates the ip= handling as documented in
https://www.kernel.org/doc/Documentation/filesystems/nfs/nfsroot.txt
